USCIS Medical Exam Form Update – New Edition Required Starting July 2025

The USCIS recently issued an update to Form I-693, the Report of Immigration Medical Examination and Vaccination Record.
If you are applying for a green card using Form I-485 (Application to Register Permanent Residence or Adjust Status), you need to pay close attention to recent changes by U.S. Citizenship and Immigration Services (USCIS) regarding Form I-693, the Report of Immigration Medical Examination and Vaccination Record.
A new version of this form goes into effect mid-2025, and using the wrong one could delay or even derail your green card process.
Key USCIS Deadlines for Form I-693 in 2025
If your civil surgeon signs your Form I-693 on or before July 2, 2025, you can use either of the following editions:
- 03/09/2023 edition
- 01/20/2025 edition
Starting July 3, 2025, only the 01/20/2025 edition will be accepted by USCIS.
The edition date is located at the bottom of each page of the form and the instructions. Make sure you’re using the correct one!
Why This Matters for Green Card Applicants
USCIS now requires Form I-693 to be submitted at the same time as Form I-485 for most applicants. Submitting the wrong form, the wrong edition, or an incomplete package can result in:
- Rejection of your Form I-485;
- Processing delays;
- Possible Requests for Evidence (RFE); and/or
- Extra costs and missed deadlines.
